To buy demo board that described in DER-286 article

I would like to evaluate your LNK419EG in our new project, to do so I think I can make use of PI’s demo board that described in PI’s DER-286 (30W universal input LED driver) article.
Can you please let me know the model number of that demo board and where can I buy it?
