low power design

Dear Sir
I am using this LNk625PG Based design but output voltage is not coming 5V. This is coming 15V and current 10ma . please help me.what is the Problem

For a 5V ,1A application of LNK625PG , you can base your design on DER 669:

Have you tried modifying your transformer similar with the design above?
