problem with thermal shutdown

Hello
I have a problem with the temperature shutdown. When we have a load on my circuit and when the temperature of the TOP250 is about 40 deg C the TOP shutdown itself. when we put a fan on TOP we don't have this problem. But I don't think so that 40 deg is a high temperature for it. we designed our circuit with PI Expert 6.6 and 7.
We designed 3 circuits with TOP250 :
48V , 5.2A
30V , 3A
12V , 10A
every 3 circuits had this problem.
Please help me.
Thank you so much for your supports.

hello, kindly attach the schematic & transformer details. what is the transformer tempreture?under what conditions & method you are testing tempreture. also which instrument u r using to test.also, pls do check, what is the maximum operating load current it withstand without thermal s/d. mail me at prasunkuls@hotmail.com
40 C is very low for thermal shutdown. Where are you measuring the temperature - the plastic or metal tab? The lowest you should see measuring the tab is 130 C (there's a small temperature delta between tab and die).
